Sick notes

If you’ve been off sick for more than 7 days

All of our 'Fitnote' appointments are assessed in a face to face capacity in the surgery.  We have specific slots made available each day for these with each Clinician.  Please contact the surgery on the designated line 01642 915851 that will be open daily from 9am until 10am. 

You will be placed on a triage list and one of the Reception Team who has been specifically trained by our GPs will ask you a series of questions to ensure we are able to assess you correctly.  

If the 'Fitnote' is related to a benefits claim then we will be asking for proof of Work Capability Assessment dates and outcomes as well as any other information that we need.  

 

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
